
FIRST ZERO CONTRAST TAVR IN UPPER NORTH INDIA

A first of its kind novel procedure Zero Contrast TAVR was done in a patient 63 years old male suffering from severe calcific aortic stenosis. Patient had severe LV dysfunction (Low EF) also patient was suffering from acute kidney injury (Creatinine 4.8mg improved to 2.0mg), severe anaemia which was corrected before procedure from 7.8 to 11.4mg%. Patient was also suffering from Waldernstrom Macroglobulinemia. Patient had recurrent admission for heart failure in the last six months also patient suffered from ventricular tachycardia requiring DC cardio version. Transcatheter aortic valve implantation was done in the above patient in view of his prohibitive surgical risk. Planning of TAVR was done with the help of a CT Scan which showed high calcium score. Patient underwent TAVR with a self-expanding valve (26mm Hydra valve). This procedure was conducted without a single ml of contrast (Zero contrast TAVR). The patient had uneventful recovery. This case is among the first few rare cases performed in patients with such co-morbidities. This is the first zero contract TAVR in upper North India. Procedure was performed by Prof Yash Paul Sharma, Prof & Head, Department of Cardiology, Advanced Cardiac Centre, Dr Prashant Panda, Dr Vikarm Kala & Dr Pranav, with assistance from Dr Abhishek. Dr Anaesthesia and Transesophageal support was given by Dr Sunder Negi and Senior Resident from the Department of Anaesthesia. Prof Sharma expressed that this type of procedure will be very helpful in patients suffering from severe aortic stenosis and renal failure.
